Do all HP ink jet printers come with replacement nozzles in the new
cartridges or is that now built into the printer head like on Epson ink
jets?

Both in fact. The HP professional series have separate ink tanks and print
heads but both are replaceable by the user (unlike Epson). The cheaper models
tend to have combined ink tanks and print heads. I am unaware of any HP inkjet
that has a printhead that the user cannot replace one way or another.

Are there any HP Inkjets that can print directly onto discs?
There certainly used to be 3-4 years ago, a quick search turned up nothing
current but your nearest HP dealer would be able to help.
HP used to provide Lightscribe (using a cd/dvd burner) which was an inferior
result (no colour) but more permanent than printable dics.
Also unless my memory fails me the patent for printing on printable discs was
owned by Epson and the technology was not available in every geography.
